Your best idea to find out if you are ready to write the exam is to go to the International Board of Lactation Consultant Examiners (IBLCE) web site.  They can also send you an application form.  It sounds like you have a lot of your requirements met.  I would also direct you to IBCLC2B@yahoo groups.  This is an online group of people getting ready to sit the exam.  There is a suggested study guideline posted by Linda Smith in the archives and discussion r/t various aspects of test taking, clarification of reading ect.
