I saw a woman as an outpatient today who was consulting me to evaluate milk supply. The baby was born on Saturday, at 39 weeks, went into labor on her own and had a short labor, and easy birth. Baby latched well in the hospital and was discharged Sunday. By Tuesday the baby was very irritable, difficult to stay on the breast, and had lost 12 ounces. Pedi recommended supplementing and parents report that that was the first time he slept more than 45 minutes. Since then mom has been giving formula by bottle, latching when he will, and pumping every two hours. The most she has ever pumped was 10cc and many times just gets a few drops. She has "B" cup size breasts with minimal changes in pregnancy. Some milk can be hand expressed and baby did nurse fairly well with an SNS, but without it he latches and has a non-nutritive suck or gets angry. She started  on Fenugreek (very low dose) with a possibility of starting on Reglan later in the week. She is very compliant with pumping with a hospital grade pump.     



Mom has an unremarkable medical history (age 35, is tall and thin, no difficulties getting pregnant), except for her condition of hyperhidrosis (excessive sweating). She had a thoracic sympathectomy (ETS) several years ago that incise the nerve that control the sweating to the hands. 



Does anyone have any insight into this condition and could the condition or the surgery be a connection with her low milk supply at one week? Could not find a cause for this condition other than genetic.
