Hi Laurie,
 
Thank you to all who have responded. My computer showed the blue screen of
death on Friday am, and both my van and car broke down on Friday
afternoon... What a wonderful weekend (and $$$!!)
 
I was looking for red, shiny, itchy, she has none of the typical thrush sx.
That's why I thought maybe not thrush, but the white spots really look like
thrush. The closest I've come to a photo to show what I'm seeing is Figure
181 in the Breastfeeding Atlas. (man, I LOVE that book!) but the white on
her nipple is across the face, and thicker in the creases.
 
With the lack of other symptoms, I hesistate to call it thrush, but didn't
know what else it could be. I'll talk with her later today, and see if she's
had any success.

I don't suspect thrush. I've been reading about wounds and infections etc on
internet tonite. Thrush usually presents on the nipple as red, shiny, itchy,
burning. I've been reading about wound exudates. I am no expert on this, and
in fact, the search reveals this is a highly complicated field (wound
assessment and care). So at first I thought you meant a white *blanched*
nipple, which if she is pumping I would think about a too tight flange or
too high suction level. But if you mean an exudate from a cracked nipple,
this could be infection or wound healing. I assume the mother had a bad
wound and that is why she is now pumping that side. Infection usually also
has pain, warmth, redness. The wound could be swabbed and cultured. If it
appears to be healing, then I would recommend gentle soap and water
cleansing and then apply a hydrogel dressing, used as directed. The mom
should also be careful with pump fit and pump suction so as not to
continually re-open the wound. Thin coat of lanolin could also be used as
moist wound healing, although in my experience lanolin is not helpful in
other than a mild abrasion, or mother's say they use it for "dry" nipples. I
am not a fan of lanolin use. I think it keeps moms from tending to a nipple
problem, they just keep applying lanolin for weeks and weeks. I do not
generally use it. 
If you have access to a wound care nurse, they could definitely help.
