Dear Ladies,

ptp

Multipara (30 yrs old) I've helped over the last 9 years with previous 4
babies.

Mom basically healthy but has suffered from Irritable Bowel syndrome in the
past.

Saturday, noon, - Intervention-free birth in Brussels, Belgium .

Baby born 40 weeks, 3.800 kg.

6 hours later Mom phoned me suffering from excruciating pain.

Deep laceration across nipples. Very strong suction. Just as she experienced
with firstborn.

The next babies were relatively problem free ( -her notes from that first
consultation kept her afloat..)

H/e this time to no avail.

The photo she sent me of the baby's upper lip shows a thick labial frenulum,
connected at least half way down gum.

In the next photo taken when crying , the tongue looks short, wide and flat
and doesn't rise into the upper half of the oral cavity.

In the video she just sent I can hear definite clicking sounds every 3
sucks// Upper lip seems to be tightly in. Externally asymmetric latch
"looks" fine.

Today she moved to an aftercare center in Antwerp where she'll be til
Thursday when she's due to return to Brussels.

The staff recommended she apply silverette breastfeeding cups to help heal
the nipples instead of APNO I suggested as per Dr Jack Newman.

The Mom's pain is not improving at all. She's taking defalgan for the pain.
I will suggest arnica.

This dyad must be seen.

Pls fwd recommendations of IBCLC's in Antwerp & Brussels and also of
professional lingual and labial releasers in the area..

Your experiences with silverette breastfeeding cups also appreciated. Pros
and cons.

Thanking you in advance,

Debbie Kali
