Dear Friends,
Hi.  I just wanted some suggestions maybe I have not thought of, and would
love to hear about other LC's experiences in this area.  I have been
working with a mother who had her baby at 32 weeks.  I don't have the exact
weight of the baby in front of me here.  Mother has been giving baby only
her milk, and has been pumping for the last 4 months.  When I saw baby at
almost 3 months, the baby had just started with bottle feedings and was
still having trouble with that.  It was a big effort for the mother, took
the baby about 1/2 an hour to finish the bottles, only with a lot of
coaxing.  As per my understanding of research, correct me if I am wrong,
but breastfeeding would not necessarily be more stressful, etc.  We did
lots of skin to skin, the baby would only latch on and actually suck with a
size small medela silicon.  I felt this was better than nothing due to the
mother needing to see some results and improvement towards feeding.  We
discussed things will be slow, etc.  Eventually we will wean off silicon,
etc,  Now it is 1 month later, and the baby is 3 kilo.  The mother is very
frustrated because she says the baby just sucks without getting much at the
breast.  She can feed for 30 min and have him get only about 10-15 g.
        What is the smartest way to deal with this?  I know of another premie
using a FF tube at breast to make sure he enough, and that mother is
complaining that the baby will  not empty the breast by himself so she ends
up pumping to empty, and then BF with the tube.
        How do we get premies to make the transition to really emptying the breast
without getting "worn out", while still monitering intake and making sure
they continue to gain?
        How do we wean of of the FF tube?
