I have learned that sometimes it helps to repeat the same question in many different ways. If you blankly asked the mother how many times she breastfeed and she said 4-5 times. Then that is how many times she breastfeeds but what she might not have added is that dad feeds the baby 2 bottles a day or the baby goes to daycare or has a home sitter that feeds x bottles a day. 
What seemed like a very simple and straight forward question from you might have lead you to a very straight forward simple answer from her - how many times does the baby breastfeed? You might have received a different answer if you asked her to tell you when and what the baby eats. Leading into bottles and/or solids. We all know that if a child receives bottles this could be the cause of fighting at the breast. 
Also I would question about the babies weight gain. There is no set rules on breastfeeding. Even though it is not the norm for a baby to remove 6 ounces from the breast at each feeding - it is still possible. There is always that one in a million. If the baby has been gaining weight fine and does not receive any other feedings via bottles, cups, spoon, dropper, sippy cup or solids then he is doing something amazing at the breast.
