I am happy to hear that hospitals are tracking formula usage through the medication distribution system.   I would also like to see an informed consent signed by the mother acknowledging all the potential hazards of formula supplements.   Mothers should also be advised to record the tracking number and lot number of formula they give their babies because of frequent FDA recalls.
