I should have added two further points to my previous posting.  The approach 
to feeding newborns is different from that of feeding a baby of two or three 
days (average)
Colostrum is produced on the basis of quality not quantity.  A newborn does 
not need huge amounts of milk and in addition he is often tired and sleepy 
after the birth effort. So it is important to feed from both breasts if the 
baby will take it and alternate the breasts even if there is a short gap 
between feeds in order to activate the production and flow.
When the milk comes in more fully after those couple of days, then the issue 
of the fat consistency is more relevant because the baby may well not take 
all the milk from both breasts and it is better to empty the first breast 
before the offering the second - if the baby agrees to this.

From the days when mothers were told "10 minutes each side," they are now 
told, "give only one breast at a time."  So I get mothers calling me to say 
that the baby has finished one side and is still hungry - should she give a 
bottle????
