I have now looked through the archives and see there has been a lot of
discussion on this topic in the past (and thanks Catherine for your reply).
It looks as though fortifying milk is a controversial topic(I did not know
there was human milk fortifier).  In this case it seems like the problem is
that the baby is not absorbing enough.  I think the high fat milk first
method may  be worth a try first?

Why the baby is not breastfeeding, I am not absolutely sure (but I do know
lots has been tried by others), but his suck/swallow reflex is not good and
its a trial to get him to bottle feed too.
