150 mg of fluconazole should be taken daily, but I think this dose may
be inadequate even though it *may* do the job.  I generally treat with
200 mg loading, then 100 mg twice a day for at least two weeks *and*
until the mother has been symptom free for 7 days.  This decreases the
risk of relapse, though it doesn't eliminate it completely.  By not
using large enough doses, we encourage resistance.

Why is gentian violet the *next* step?  I think it is the best *first*
step.
