I am writing a paper about supplementation of the full-term healthy
breastfeeding baby for my final MSN project. 
For those who work in hospitals, would you say that the main reasons you
encounter for supplementation are:
1. maternal request
2. weight loss over 7%
3. hyperbilirubinemia
4. hypoglycemia
5. other? 


Would you say that maternal request is due to?
1. lack of information about normal newborn behavior
2. concern over sleepy infant not eating
3. concern over baby eating "too much"
4. cultural concern over having "no milk"
5. wanting to sleep and let someone else care for baby
6. encouragement by nurses to "help" mom through a rough patch
7. painful nipples 
8. engorgement
9. separation due to mom going for tubal ligation or blood patch
10. family encouragement to feed baby bottles

Please feel free to add to my list. 
What percentage of babies do you think leave your hospital without being
supplemented? I would appreciate any help.
Also, I need some good references written within the past five years
regarding supplementation. I have a few from JHL.
