To Julie Horwitz:
There's an recent article that you might want to obtain. If the LC working with
this mother is hospital-based it ought to be easy for her to request it.
(Thanks to Lisa Marasco for mentioning it on Lactnet a few months ago.)
"Induced Lactation: A Study of 37 Non-puerperal Mothers," Kuria Nemba, _Journal
of Tropical Pediatrics_, August 1994, Vol. 40, pp. 240-242. The protocols they
suggest seem very reasonable to me (Dr. Craig's induced lactation protocol with
all the estrogen patches makes me nervous, but that's me) and they report good
success. I haven't had an adoptive mother since I read the article, so I have
no personal experience with the protocol yet.
Basically, the procotol consists of:
For mothers who had never lactated: A single priming dose of IM Depoprovera,
then one week later either chlorpromazine 25 mg 4x/day and/or metoclopramide
(Reglan) 10 mg 4x/day until adequate lactation was established.
For mothers who had lactated previously: Either chlorpromazine or
metaclopramide, same dosage, until adequate lactation was established.
For all mothers: lots of infant suckling and support.
