To add to the sore nipple discussion:

I saw two moms last week - both were referred to us (Public Health Nursing)
by WIC due to sore nipples - unfortunately, these moms saw WIC 3 and 4
weeks after delivery, so by the time I saw them it had been a few weeks of
breastfeeding that they had suffered through...

And I do mean suffer - they assumed it was normal to have pain with
breastfeeding, so they never called, just bit their lips and kept on
breastfeeding.  

By the time I saw them, babies had developed their own way of
breastfeeding.  It was VERY difficult to get baby to take more than just
the nipple.  Though mothers could see how different it felt when baby was
correctly positioned, babies would push the nipple out until it was placed
where they were used to sucking.  Repositioning sometimes helped - but many
times not.  I did do some suck training on my finger (I know, babies learn
to breastfeed at the BREAST - but I was desperate to try something...), and
found they did latch on better for a while - but when put to the second
breast, they returned to their usual pattern.  Any suggestions?  Even if I
can't help these two moms, I'd love to get more tricks up my sleeve for the
older baby.

I'm trying to develop something that WIC and our prenatal programs can give
moms when they see them prenatally that will trigger calls earlier -
besides the number of wet diapers, stools and weight gain (both babies were
doing fine on that score), would you add "if you count to 10 after the baby
latches on and it still hurts, please call"???  Or would you say "if the
pain is greater than ___" please call???

[ Yes, we know it is a big problem that they are referred so late - the
Breastfeeding Task Force has identified this as a problem, but the
solution, when staff at hospital is not referring  and niether is Home
Health (see my previous note), is something we are still trying to work
out.]
