Very interesting post.  Goes to show that there's rarely a single
answer to a breastfeeding challenge.  There was one small point I
wanted to comment on:
"Also, new and interesting, baby is refusing the larger (presumably
more
productive) breast without the SNS."

Do you mean without the SNS in place or without it "turned on"?  I
wonder if baby is so used to the feel that he figures if he doesn't
feel it it isn't working right.  (one of the potential complications
to its use).  Has she tried having the tubing in place, but just not
delivering any milk through it?

I would definitely ask about surgery AND any previous injury to the
chest wall.  However, since the baby acted the same when you nursed,
I'm less likely to think that's a factor unless he's established
that pattern and may need to learn it's not necessary when there's
more milk available.

Do keep us posted.  I do so admire the moms who are willing to work
through the process of trial and error as opposed to the ones that
quit the first time baby pauses at the breast!
