I don't think that hind-hindmilk would compensate for the minerals and
protein needed by the premie infants. It does provide extra calories and in
small volumes.
I do think that more research would be informative in that liberalizing
volumes might be an option.
Over the years premie nutrition has changed in that we used to
keep babies on just IV fluids longer and now "trophic feeds" are done, and
babies are started feeding sooner. We also used to dilute breastmilk 1/2 and
1/2 with sterile water to start with, which is no longer recommended.
Manufacturing fortifier from donor human milk is a new step in the science,
and I think a good thing, although there is some concern about the ethics of
making profit from this process.
Most, if not all of this, has been discussed here before, and you can find
alot more info and opinions in our wonderful archives. I hope Dr. Wight will
respond to this too, as I do not regard myself as an expert in this
field, in any way.
