Well, Robin, you force me to declare my self-interest in all of this HIPAA
stuff.

When HIPAA first came out (2004) I, as a private practitioner, went nutty
because there was a lot of confusing information out there. I was hoping,
frankly, to find a loophole to declare that little ol' private practitioners
like me weren't covered by the regulation. Then I started reading the
regulation (about 300 pages in the Federal Register, all with
phonebook-sized print), and looking at the materials that were being offered
to HCPs, and I was eye-rolling at how unnecessarily obtuse all the paperwork
was. I am a lawyer who firmly believes that legalese does *not* have to be
complicated, and I have used plain language all of my professional life.

So -- I went off and wrote a package of commonly-used HIPAA documents, for
sale to the private practice lactation consultant to use in her practice. I
threw in a consent form and fax cover sheet too. I welcome any inquiries
off-list.
