I recently saw a baby with this problem, and decided to try a latch-on
technique that I heard about at Diane Wiessinger's presentation. Instead of
tickling the baby's lips with her nipple, the mother uses the breast below
the nipple to tickle/rub the baby's lips. The curve of the breast seems to
encourage a wide-open mouth with tongue down and forward. When the baby
opens wide and his tongue is down, the mother then guides the nipple into
the baby's mouth and it naturally goes right on top of the tongue. It worked
very nicely and the mother was thrilled!
