Metformin is used to reduce glucose levels in non-insulin dependent diabetics - usually Type 2 who have insulin-resistance. PCOS patients often have Type 2 diabetes and it MAY be the underlying insulin resistance that triggers the PCOS.  High androgen levels are associated with PCOS which may affect milk supply and fertility.  Giving Metformin to folks with Type 2 diabetes is a good thing and may treat the conditions that underlie low supply.  However Metformin treatment is also associated with lactic acidosis and women taking Metformin should not be consuming alcohol.  Some of the non-medication treatments for PCOS - including nutrition, exercise and weight loss when required, may be helpful. But using Metformin as a go-to medication for decreased supply would not be my first option.  Newman reports that domperidone may have less effectiveness later on in lactation.  We really don't know what are the best strategies for improving storage capacity in the breast and this may be a real issue for some women with low supply.
