In my experience, many babies won't suck on a nipple with air coming through
the hole (or a breast with an empty SNS) but, if the baby doesn't mind
sucking air, it is no problem.  I don't think they swallow a significant
amount of air that way.  At the time I was weaning Julia from the
gastrostomy and teaching her to feed like a normal baby, which eventually
led to breastfeeding, she could not take more than a very small amount of
food at a time, because of her small stomach (due to diaphragmatic hernia).
I would just put a small amount in the bottle at a time, and then let her
suck however long she wanted to, which was sometimes quite a while.  Had she
been swallowing alot of air, I think it would have been a problem for her,
between the small stomach and the fundoplication that made it difficult for
her to burp.  Actually, they said she wouldn't be able to burp, but she has
no problem now!  At 13, she can keep up with her four brothers, when it
comes to belching contests!
