Below is a past post from Cathy Genna I've saved that may be of help.  The 
response for CST sounds good and perhaps an OT or SLP could help.  I've 
suggested below to moms but follow through is minimal, so don't have experience with 
it working.   A nipple shield might temporarily help if baby is rubbing the 
nipple, but not if clamping.  Is this mom sure frenulum isn't restricting?  Who's 
evaluated for this?  Could be the often missed posterior ones.  How has this 
mom managed for 3 mos with nipple pain/damage?  Poor mom.  You also mentioned 
abundant supply, might prone positioning help?  Side lying so baby in control? 
 Letting baby be in control worked recently for mom/baby who await frenotomy.

"Occupational therapy trick for hyperactive gag: Tickle baby's lips so 
he opens voluntarily. Slide finger to palate, press firmly just behind 
the upper gum ridge (the very front of the hard palate), wait until baby 
relaxes, slide finger back further along the palate as accepted. If 
child starts to gag, retreat to gum ridge again and press strongly.
Obviously one wants to use this sensitively, and teach parents to watch 
baby's cues to make sure that it is being accepted and tolerated.
