It sounds like there might be some structural issues (asymmetrical, 
popping jaw) and perhaps some neurological ones. The neuro issues can 
certainly be caused by pain, which disorganizes and dysregulates babies; 
or the sucking issues can be caused by a minor neurological problem, 
which leaves the baby a bit hungry, which increases the dysregulation.

Mom is observant, the excessive lip movement is compensatory, and when 
baby is more "together" he can do a better job with his tongue and does 
not need the lips as much. The sliding down to the nipple bothers me, I 
almost always see this with those sneaky posterior tongue ties, or a 
short mandible that changes the relationship between the jaw and 
frenulum. Sometimes you can't see anything, until you slip a finger 
under the tongue and lift it.

With a posterior tongue tie, some babies improve markedly between 6 and 
12 weeks, even without intervention of any kind. Some continue to cause 
pain until treated with frenotomy.
