I have personally worked with three women who have been able to breastfeed
without supplements after breast reduction surgery.

One was kind of interesting - the mother decided to have the surgery while
breastfeeding her first child. He was about three months old at the time of
surgery. The surgeon told her she must wean first, but she kept putting it
off and finally it was the day before the surgery and she was still nursing.
Afterwards, the doctor told her that in fact it was helpful because he could
easily see the milk ducts and tried not to damage any of them. He told her
not to try to nurse the baby again because it might pull out the stitches
and gave her medication to dry up her milk. She ignored that recommendation,
too, and put the baby back to the breast within a few days and continued to
breastfeed with no problems. She had two children after that and both nursed
for long periods of time with no supplements.

Another mother had breast reduction surgery at seventeen, married and had
her first baby at 21, and had no problems producing plenty of milk. Didn't
do anything in particular, just helped her get a good latch and nurse on
cue.

The third had needed to supplement while breastfeeding her first baby, but
had a full milk supply for the second baby.
