I have one phone line, I am both LLLL and IBCLC,   and I do the same as Pat.
Depends on the referral source.  Sometimes it isn't always clear how the
mother got to me, (when referred by a friend, for instance), but usually
this can be clarified through a few questions. As LLLL's we are not allowed
to refer to ourselves as IBCLC's.   So, the problem is when someone calls
for LLL, but has a problem that clearly needs to be seen by an IBCLC.   I
need either to refer to another LC, or refer her to another LLLL who will
then refer her back to me as an IBCLC.  The latter is the usual scenario,
since I am the only LC in the near area.   In these cases, I explain the
situation to the mother, and tell her that I cannot see her as a IBCLC (and
charge her), and I leave it up to her to decide what to do, (to call the
other leader for a referral back to me or not), but upon explaining the need
to do this to the mother, this seems rather complicated, and since this is
almost always a new mother in the midst of a  breastfeeding crisis, she
can't be bothered with these formalities and will almost always prefer to
schedule a paid visit with me during that first phone call.
This is problematic, and had been discussed often here among those of us who
are *both*.
