33 yr old primip.  I saw her on day 8.  Vag delivery @ 39 wks.  Mom on Nifedepine for hypertension.  Baby was re-admitted to hosp DOL 4 with dehydration, hypernatremia, hyperbili.  BW was 3230, 2810 on DOL 4 and 3088 when I saw him.  Mom has absolutely no milk.  No significant history to speak of.  OB performed D & C on  day 9 to r/o retained products and started her on Reglan QID.  She had horrible vomiting and diarrhea and had to cut back to BID and added Fenugreek.

Mom is putting baby to breast 8-12 times in 24 hrs and pumping 6-8 times with a Lactina.  She occasionally gets a drop or two from the L breast, NOTHING from the R breast.  She is trying heat, massage, everything we can think of.  What have we missed?
