Hi Felicia,
I worked with a mother about three years ago with a double nipple.  She had a
closer configuration, with the nipples nearly touching.  Pumping was not
comfortable even with a larger flange, but the baby was able to accommodate
both nipples simultaneously.  I only saw this baby in the hospital, so cannot
give any long term results.  It seems to me that trying to latch on only one
nipple would cause the other nipple some discomfort from friction.  With the
mother I worked with, both nipples were functional, milk was expressed from
both.  The "regular" size nipple can be quite varied, so at this point a
large mouthed, vigorous, non-epiduraled infant may be the best hope.

It looks almost like she has 2 small nipples.  One close
