Hello everyone,

I have been working with a mom who is struggling, and I need to run this
past you all to make sure I didn't miss anything.

The baby is 12 weeks old, was gaining well, though they haven't weighed her
in several weeks.  Urine output high, bowel movements regular and mustardy.

Baby has been very "colicky" all along, shrieks.  Several weeks back baby
would be very upset, stomach distended, very gassy.

Baby spits up a lot.  Like, 8 times after each feeding, and several hours
after a feed - even stops nursing to spit up.

I've worked with the mom some on the latch, though it's not too bad.  Baby
comes off easily, though, slips off a lot during feeds.

Baby has no pacifier, no bottles.  Mom nurses on demand, but baby wants it
steady and if she stops, the baby shrieks.  If she doesn't stop, the baby
spits up all over her and then keeps nursing - sometimes for several hours
on end.

Over the last month she has started nursing on one side per feed for up to
four hours.  That hasn't helped. Baby nurses slowly and lazily, no sign of
heavy gulping or having trouble with flow.  Mom still hasn't picked up on
baby's suck/swallow pattern, can't seem to hear the swallow but is going to
work on this.

The doctor has ruled out pyloric stenosis and reflux.

She just suffered a bout of mastitis, and now she has signs of thrush that
she is actively treating.

Baby is having vitamin d drops, but mom has stopped this for a couple of
days at a time with no effect.  Mom is taking half dose of prenatal vitamins
but has tried ceasing this for a couple of days and that didn't help.

The baby doesn't sleep much - very alert.  But also very unhappy.

We discussed normal nursing patterns and variations.  That it is okay to
nurse the baby more if that offer sanity to everyone.  (We touched on 'bad
habits' and her fears of setting patterns.) Temperaments.  That this is the
fourth trimester and some babies just need extra time getting used to the
outside world. She has been using "The Very Best Baby" with a little
success.

We've discussed trying to eliminate dairy, though she already eats very
little, and she might give that a try.

Mom and Dad are very tired.  Mom is easily moved to tears.  I suspect she
feels she is doing this "wrong" since other mothers seem to actually enjoy
parenting.

So, what did I miss?
