I could offer some help here.  I could code the data and enter it into SPSS
(a statistical program).  I just had experience coding and entering 1134
surveys into this program.  I am not offering to analyze the statistics (it
is not really my expertise and the information will be too valuable to mess
up), although I could do descriptive stats.  We would have to use the same
form for everyone, otherwise the data would be impossible to code.  We would
have to put together the form and test it to make sure it is valid.  This
sounds like a great study.
