Judy,
I worry about your mom with migraines lapsing into a rebound situation by using the butalbital combination. She is probably on the outside edge of OK using twice weekly, but any more can lead to rebound headaches. Butalbital is notorious for this sort of thing. Imitrex would be a better choice.

Nursing during a migraine can be a lifesaver when you are incapable of taking care of the baby otherwise. I had a good friend (co-leader) who latched my baby on to me, then turned the lights out and locked the door as she left after a meeting. If the baby had not been nursing, I don't know how I could have cared for him. Napping with the baby would be a good way to get some sleep, as she is unlikely to have a regular sleep schedule under any circumstance.

I would also suggest that she be careful with caffeine intake, and position herself carefully so she keeps her head in a neutral position. She probably does need some prophylaxis for migraine to avoid rebound, but should be working with a really caring physician -hopefully headache specialist - to help her sort it all out. Many of the prophylactic meds can really make you feel tired, and some can make you pretty drowsy. All the more reason to get good help.
