I have a mother with VERY LARGE, wide, fibrous nipples and a baby (now
four weeks old) who is refusing breast...long story. The Medela 24mm shield
is too  small.

~~~ Hi Angela,
Medela makes a 27, 30 and 36 mm...you can check their website or try a
local store that rents and sells their products.
Peace,
Hi Judy,
I believe Angela is talking about the Medela Nipple Shields and that you are talking about the sizes of the Medela Breast Flanges available.  I searched Medela to be sure and the 24mm is the largest size Nipple shield that is available.
I wish there were larger nipple shields.
