No one yet has mentioned in the course of this discussion that the possible
problem with using a sucrose pacifier for pain relief (for simple procedures,
etc) is that *it is a pacifier*!
  I was reeling just this past week trying to help a mom with a two day old
non-latching baby who was given exactly ONE pacifier (while his mom was in
the shower and expressly against her wishes) after 24 hours of beautifully
nursing .  And because of that pacifier, baby refused to latch on for the
next five hours.  The nurses had badgered the mom so badly that he needed to
be fed that the mom allowed a bottle of glucose water to be given at 1:30pm.
Mom was discharged that day (3pm) with baby who had not breastfed since
8:00am.  I saw them in their home at 6pm, a rare home visit for me, and baby
was close-mouthed, sleepy, and tongue thrusting.  He would "latch-on" and
slip down on the nipple to suck on it (ouch!)  He did what I call "checking
out"- pretend to wake up, opened his mouth lazily, had no clue what to do
with his tongue or the breast, and promptly fell back to sleep.  His mouth
was dry and his cry was hoarse ... and his mom was very angry and upset at
this situation (she is an experienced, third time breastfeeding mother).
  The bottom line for me is that "just one pacifier" is not always "just one
pacifier".

btw, on follow-up two days later, baby was nursing great!
