Despite this mom's breast surgery and previous poor-gaining baby, I'd look
first at the baby.  Is there any evidence of tongue-tie or tight labial
frenum?  The baby who can't attach well will trigger a let-down only when he
manages to hit the right spot - which isn't every time.  I've had several
tongue-tied babies whose moms were more concerned about their erratic
let-downs than about any soreness.  In each case, after the tongue-tie was
addressed the MER worked.
