Has this mother's pregnancy situation changed in any way? I would presume the OB would know if fetal or placental health is impaired.

Has she perhaps been put on a diuretic? Has she perchance been put on bedrest?? Swelling in pendulous breasts can be due partly to edema even during pregnancy. Is she by chance doing any breast massage for comfort? Both direction of gravity and massage can help lymphatic system to deal with any edema.

Just a few thoughts.
