This is the first time I am writing to you( have been reading on lactnet the 
last half year)
I am needing help on the following situation(permission to post)
I saw this mother the first time on august 1st, very emotionnely, because 
she stoped breastfeeding at july the 14th due to severe cracked nipples. Her 
breasts were still leaking every time the baby cried and she really wanted 
to breastfeed and would like to restart. She still had cracked nipples at 
that moment(really on the point of the nipple, the tissue was gone, lying 
deeper- even though she didn't breastfed or pomped her milk for over 14 
days). She hadn't been pomping because even pomping was to painfull.
This women gave birth on the first of july, she had a waterbirth (so no 
anesthesia), breasfeded her baby in the first hour after delivery, had her 
baby al the time with her, terme=372/7 weeks of ammenorhee
She is allergic, has asthma (ventolin)
Baby is a little boy,2440g at birth
It's her second child, first baby was born by cesarian, breatfeeding 3 
weeks, stopped for severe cracked nipples.
Mother, first child and this baby have all sligtly regressed chin
Severe cracked nipples in the first few days, went home like that, came back 
a week and a half later:mastitis, infection with staphylo, got erytromycine.
Since the pain went on she went to her generalist and asked for a treatment 
for candida, which she did for 2 weeks, for her and the baby
On the first of august we started Motilium(domperidone) 4times 1 a 
day,pumping at least 8 times/24h, put some milk on the cracked 
nipple/lansinoh.
The baby was hungry, tried to latch on but was to nervous, since her nipples 
were still sore we agreed on pumping
Pumping isn't going so well for this women to stimulate,neither with massage 
or manuel expression
Therefor she put the baby again to the breast, and it seemed to work(nipples 
didn't get any worse, milkproduction increased= 50g/time pomping)
untill last saturday: again fever, less milkproduction, red diffuse spot or 
right breast, extremly sore nipples(advised here to put bactroban on her 
nipples, stimulate her breast and put some white cheese on the red spot 
after each pomping)
I saw her again yesterday: the craks seems the same, but are more painfull, 
baby on the breast non nutritive sucking, and pain for his mother
The baby has no tongue-tie, normal palate, normal suction on finger.
Baby and mother were seen by osteopathe= OK
What can be the problem here??
