Jennifer,

This family seems to be a great candidate for reverse scheduling. To do this
they would purposely bring the baby to their bed (either in their bed or with
a sidecar/co-sleeper) and increase nursing during the night to increase total
daily consumption. This would not require her to have to change her current
daily pumping schedule.

I would like to emphasize that, for the family's sleep, the baby will need to
be in very close proximity with the mom so she can just snuggle to latch and
go right back to sleep. I always explain the beauty of synchronized sleep
cycles and how the baby is perfectly "taught" how to fall back asleep while
nursing. It would be too exhausting if she tried to get up repeatedly if the
baby is in a separate location (even a crib in the same room is too far).

Another option is take her lunch hour to go nurse the baby at the daycare.
Or move to a new daycare closer to work to accomplish that. Have you
explored whether she nurses just before leaving the baby and immed. after she
picks up?

You could also explore with the family the chance for the mom to move down to
part-time work or take work home so she can be with her baby more--for the
next year or so. This would, of course, help in all areas of development as
well as protect the long-term health of this allergic child.

Families in your practice sure are lucky to have you!
