Hi All,

 

Ok - assuming I am not as up to date as I'd like to be on how prolactin
levels work (I am assuming this and I'd probably be right as it is an area I
don't tread in too regularly :-) ) Jean made some interesting points and I
would love more info on this as well.  :-)

 

So - when did mom have her levels drawn in relations to BF?  Not sure - but
I can say that it probably wasn't drawn at the different times that Jean
suggested.  In fact most times I ask for levels to be drawn parents get them
done well before 10 days postpartum - so if I see an 84.5 on day 5 I know
something has to be out of whack.  I usually ask for levels to be drawn when
there is little to no milk for no other apparent reason - sadly this mom has
lots of other apparent reasons to go with her circumstances not all of them
hormonal.  And I believe I did mention her age - 41 first time mom - no
fertility issues at all.

 

So my questions are:  is prolactin safe to take given her circumstances;
should I refer (already have) strictly to the endocrinologist and not offer
any more information on herbs (as galactogogues) or domperidone (unless Endo
says they are OK); should I tell parents there is no hope of building a full
supply - remember mom is non-compliant for the most part;  is there hope???
At this point I am just feeling over my head with what to tell parents that
is reasonable and accurate.  I have told dad we need more information.  I am
mostly supporting him as I have not spoken with mom for several days - she
refuses to talk on the phone.  I know mom is suffering from PPD at this
point - dad knows too - and I hesitate to say - "No hope for milk" as I
think it might send her into a tailspin.  However, I know that if she
doesn't do what she needs to there is no hope.  I can talk to dad and tell
him the 'scoop' honestly - and I have been making really clear to him that
"if she doesn't do "this" then "that" won't happen" and "she needs to pump
in addition to BF the babies since they are NOT transferring milk and the
pump is".He is doing the best he can to encourage without overwhelming
her.luckily they have been together for 21 yrs.

 

I am going away for a couple days to the Wolf and Glass conference but all
my clients know they can still call and I'll get back to them.we'll see how
they do.  Anyone else going to the W&G conference in Berkeley, CA?  If so -
e-mail me so we can meet-up!  :-)
