Several dentists in my area, including Greg Notestine who wrote the JHL
article a few years back, do it Soooo simply.
Greg has the mother hold the baby in her lap in the dental chair. After
examining the baby's mouth (and evaluating the frenulum using Alison
Hazelbaker's ATLFF scale), if it needs to be cut, he just leans the chair
back a little further. Mom holds the baby and the baby's arms, someone
usually gently holds the baby's legs, he holds the head with one hand and
uses a sterile scissors with the other to clip the frenulum. No anesthesia,
no clamping. As soon as it's done he sits the chair up again and mom puts
the baby to breast immediately. This comforts the baby and mom gets
immediate feedback of less pain and more effective feeding.

Some babies sleep through the whole thing. Some hate to have their head held
still or mouths examined, regardless of the clip. Only a few bleed, and then
only a few drops.
