I am anxious to hear any other ideas for this mom who has given permission to post.

Breast pain Rt breast 
   Nipple pain with nursing extending  breast pain that goes from pressure to  aching to sharp, shoots   toward axilla
   No fever, no redness, no shininess or white film on nipple. 
   Had a mole on that breast  removed PP (benign) but the symptoms were there before removal.
   Once, while pumping she pumped very red bloody milk, but that only happened once. 
   The pain is intermittent and not only associated with feedings :  she can breast feed    comfortably if there is no pain prior to the   feeding.
    Pain is worse after the feeding .
    
HISTORY:
12 weeks PP, first baby
Baby nurses 8-10 times a 24 hr period 
Baby is a good feeder/ growing well.  Baby has no signs of yeast. 
Mom is back to work part time (4-5 hours at a time and pumps) since 6 weeks, pain began before this 
+ beta strep and got abx in hospital- no abx  since
Used a nipple shield for 3 weeks, then baby on breast only. 
Has treated with round the clock ibuprofen with no change 
Heat makes it feel better but it does not go away
No signs of over active letdown. 
 
 
Had a breast ultra sound with normal findings.
Milk examined for leukocytes and bacteria and findings were normal.  
 
I  wonder about ductal yeast but she is not "classic" for yeast.  Treatment would be on pain symptoms alone.
 
Interestingly, she went for 3 days, vigorous feedings on unaffected side and snacks and pumping on the affected side and she had no pain for 3 days,  then on the evening of the 3rd day she had the worst episode of pain she has ever had.  
I am not sure what to suggest next.  Any ideas welcome.
