Pretty much what you said and did.......In these situations, when the mom and
baby are coming in for an lactation consult, I ask the mom to bring EBM if
available, or ABM otherwise, because we are going to feed the baby.
Even if my tricks and techniques work to result in a good feeding in the
office, with an increasingly jaundiced, most of the time sleepy-not-latching
baby, a >10% wt loss needs to be corrected ASAP. If she is not coming in to
see us soon enough, she should feed the baby in whatever way she feels most
comfortable at home and we will reassess method of supplement as part of the
consult. If bottle nipples are used in a situation where supplementing is
necessary, and a nipple confusion or preference results, we will fix that
later.
