One possibility might be that the milk sinuses in that breast are
somewhat deeper than in the other breast, so that the standard size pump
flange might not reach them and drain the breast as well during double
pumping, leading to a reduction in supply due to reduced removal.
Fenugreek won't help that side unless there is regular, thorough removal
to rebuild the supply.

You might want to assess whether there is a difference between the two
breasts in where you need to compress with fingertip expression. Or you
might just obtain  several sizes of larger flanges and try using them on
that particular side for a while to see if this eventually helps. The
size of the nipple itself is not necessarily totally indicative of the
most efficient size of pump flange to use. The relationship of the inner
ring of the flange to the location of the milk sinuses during the cycle
has a definite effect.
