Katsumi Mizuno found that coordination of suck:swallow:breathe was 
better in infants bottle fed human milk than in those bottle fed (in the 
same bottles) with formula or water. One would think the water would 
flow fastest and create the most difficulty, but that was not his result.

So this little one probably still has some immaturity in the suck-swallow-breathe mechanism and is finding the thicker, slower flowing fluid easier to manage. Like Judy, that would be my first guess. And I would start with trying the things Judy recommended, first and foremost observing mom doing a bottle feed and then giving her some pointers and probably a slower flowing nipple. And as has been mentioned on this list before, you can't rely on the packaging to tell you which nipples are truly slow flowing.
