Here I am again, :-)

 

Case 2 is a combo of 3 moms I am working with.  All 3 have tender nipples (1
may have thrush, 1 has been battling thrush).  All 3 have used a nipple
shield - all 3 have had their nipples made even MORE tender because the
nipple is getting pulled so firmly in the shield they are left with 4 little
'blisters' where the holes of the nipple shield are.  One mom had blisters -
they went away, the other 2 thought they were blisters - but realized that
the tissues was being pulled so firmly as to get sucked through the holes.  

 

2 moms are having difficulty with baby getting enough milk from using the
shield - latch has been worked on - it is deep and appropriate.  We think
that there is simply no way for the milk to get out because the nipple is so
firmly against the shield.  

 

1 mom quit using the shield and played tough love with her baby and they are
doing better (although this thrush won't go away).  Mom 2 has a baby who
won't latch without it and mom 3 has a baby whose hard palate comes down
sharply forming a ridge at the back of the mouth which causes her tremendous
pain and nipple trauma when he does nurse.  (I have only felt 2 of those in
the past 8 yrs - odd.)  With the shield there is no pain - she was getting a
scale to check on intake with the shield when I talked with her last night.

 

Thoughts on this one?

 

Lastly - I have 2 moms battling chronic thrush.  Both have done Gentian
Violet, APNO (over the counter), Mom 1 has done 2 rounds of Diflucan (Mom 2
can't afford it).  They have sterilized everything in sight, treated
everything and everyone they could.  Mom 1 has also taken herbs that fight
yeast (GSE, acidophilus, pau d'arco.).  No luck.  Both moms have had
problems in the past with vaginal yeast and getting it to go away.  Both
moms do fine while treating but within 4 days of stopping treatment - the
thrush flares again.  Babies have been treated (both were diagnosed by Ped
as having it) with GV and Nystatin - but Baby 1 seems to be following the
same pattern as mom - Baby 2 is not on the breast (mom has large nipples and
baby cannot yet manage the size).  All latch issues have been ruled out as
cause of pain - both moms report relief when treatment is started again.
Neither is using GV more than 4 days in a row and are giving at least 4 days
in between.

 

Thoughts here?  I am at a loss - maybe it is just the ridiculous heat - but
both moms have been battling this for 4+ weeks now.
