Poor mom, no wonder she wants to quit! I would encourage her to clear
the mastitis by frequent pumping before she stops bf. I have found that
when mothers get recurrent mastitis, either it is being inadequately
treated (wrong antibiotic, say amoxicillin!) or the mom has a surface
infection of the nipple that is not responding to the systemic
antibiotic and keeps reinfecting the breast. In this case, the mom
needs both a topical and the systemic drug. I remember one client who
had constant repeats of mastitis until she finally was prescribed some
bactroban to clear up her nipple.

Once she is clear, she can start gradually reducing her pumping. I
generally recommend mothers alternate between stretching the time
between pumpings and reducing the duration of pumping until they are no
longer uncomfortable. This can take anywhere from a few days to a few
weeks, I would encourage a mom who has had recurrent mastitis to wean
slowly.

As for drugs to suppress lactation, I only know one doctor who has
prescribed bromocriptine since the FDA revoked approval for that
indication due to strokes and other potentially fatal effects in mothers
taking it.

Good luck to her, and to you as you try to help.
