I have used nipple shields in our hospital clinic and in the hospital
setting but not until after the first 24 hours. i also start these moms
pumping. This keeps the baby at breast, without nusing a bottle. If the Dr.
doesn't like cup feeding, I'll use a lactation device at the breast with a
mixture of colostrum and about 10cc'c of water which was washed down the
horns of the pump so we don't lose any of that valuable colostrum. I have
seen colostrum in the shields even on day one if the shield is applied
properly. They are than followed closely in the clinic and are usually off
the shield within a few days to a few weeks, depending on the initial
problem.
