BFHI assessors are trained according to the guidelines from WHO/UNICEF, at
least in Europe.  They must be knowledgeable about BF and they must be women,
as they need to be acceptable on wards to go in and interview new mothers,
watch them breastfeed, and talk to them about it.
In Sweden I think people were allowed to assess the hospitals they were
employed in.  In Norway we had to be external, and none of us were involved in
the evaluation process in our own institutions, though many of us were heavily
involved in the work of preparing for evaluation (in-service for staff,
development of policies, etc.)

I don't know how it is done in the US.
