Hi Mary,
I don't think you are necessarily missing anything. Mothers of preterm
infants are at risk for inadequate supplies. I would assess her pump use (is
she using speed and suction appropriately), flange size and fit, frequency
and duration of expressing. I would suggest she combine pump plus hand
expression with each session, keeping the time manageable, maybe 20-25
minutes total. I'd rather see a mom pump 10x for 10 min, than 6x for 35 min
for example. Some moms tell me they are pumping for an hour.
Encourage her to visit and kangaroo her baby, pump after kangarooing if
possible. Help her to de-stress her life, brainstorm about her routines,
travel, maybe give her another kit to make cleanups easier, ideas about
healthy fast snacks and drinks. I definitely would suggest galactagogues,
herbal or prescription. I wouldn't wait any longer on that. She may need to
vent her fears, you may have to help arrange a family meeting if that is
needed, so mother feels a part of the team, and her fears and worries are
addressed realistically. Don't do this all at once, maybe talk to her each
day or meet with her daily.
Haven't heard of the canned corn galactagogue, perhaps the physician was
trying to give a sort of placebo suggestion.
