I would recommend a hospital grade pump, but not the Lactina, which is
widely available, but I do not find it gives good results, IMO.
I would recommend 8-10x pumping per 24 hrs, if possible.
If she can get baby to do any suckling, that would be excellent.
Also do as much skin to skin and co-sleeping (not necessarily bedsharing) if
possible.
I would recommend mom start on domperidone or reglan, as I have taken to
recommending this earlier, to the mother and her doctor, when there are
indications of reduced supply, and I am seeing promising results (anecdotal,
not research based).
Depending on mom's financial situation, if she can add herbal preparations,
I would refer her to Motherlove for a tincture or to a store that sells
capsules of fenugreek.
I would encourage mother greatly in this endeavor, as many infants and
children have had measurable improvement in similar situations, with even
small quantities of mother's milk each day.
I commend her and her caregivers for initiating and supporting this strategy
and hope to see more women relactating.
