Start on lecithin asap.  Be sure she is not anemic.   I would look for
something mechanical - like large breasts that sag and impede milk flow.  Be
sure her bra doesn't constrict the breast in any way. Or a repetitive motion
of the upper arms that cause mom to bump breast.  BFAB also mentions
sleeping on stomach!  The new Breastfeeding Answer Book has excellent pages
on  mastitis and reoccuring mastitis.  Maybe some one of the causes listed
there would be an "aha" moment for this mom, something she is doing that has
been cited as a cause of mastitis.  They quote one study that showed an
increase in mastitis if nipple shields are used and we know that damaged
nipples are  a cause of mastitis so you're stuck for a good answer on shield
