Does the upper lip look like it is trying to milk the breast, with
almost a peristaltic action from the base of the lip toward the free
edge?  I have seen this type of excessive lip movement as a compensatory
movement when babies are not using their tongues properly, whether from
a tongue-tie, or just a shallow latch that destabilizes the tongue.  The
nipple pain comes from the nipple being in the front of the mouth, where
it gets compressed by the tonguetip and jaws.
        When latching this baby on, try extending the baby's head slightly and
leading with the lower lip to increase the amount of tongue contact with
the breast, and teach mom to pull the baby in to get a deep latch.  If
this is the problem, it can take a few days of concerted effort until
the baby stops trying to use the lips to do the tongue's job.
