I'm not sure whether you were looking for suggestions or simply sharing the
case study but I am wondering if you tried a nipple shield.
 I have had a couple of cases with similar presentations. My most recent was
a 3 wk old w/ a definite br aversion, everytime we put her to the breast she
screamed. Mom had been pumping and bottle fdng. I encouraged lots of skin to
skin, including cobathing and worked with her to develop a plan which
included starting the feed with a bottle but skin to skin and very close to
the breast, feeding until somewhat satiated then switching over to the
shield. The baby would usually resist the shield , halfheartedly, at first
but then settle in once flow was established. The same scenario of *steps*
can then be used to remove the shield (eventually!) as well.
