ON Sat, 1 Jan 2011 Jennifer Welch asked for advice and support, for working
with her first client.

Jennifer, my opinion is that you have done very well in what you have
achieved with this mother, and how you have gone about achieving it.  It
looks to me as if you are fitting the nipple shield perfectly adequately, as
evidenced by the fact that this baby is transferring milk (and has lots of
bladder and bowel output) and isn't causing the mother pain.  I learned long
ago that it isn't how it looks, but whether the baby is drinking and the
mother isn't having pain.
After such a complicated start, it can take a little while to turn the
corner - and look how he is drinking and filling his diapers, even on Day 6.
I wouldn't add other suggestions, which would just complicate what you and
the mother are doing.  I'd keep on with the s-2-s, I'd continue with the
nipple shield (fitting it the way you describe), and I'd offer the bare
breast at some stage each day.  I'm pleased you are monitoring the baby for
torticollis, just in case, and that you are following through on whether
they might be a tongue tie.
You are doing great.  The mother is lucky to have you for her IBCLC. This
baby has had his head hurt and his gut given hard-to-digest substances, and
he is doing the best he can.  The s-2-s and the laid-back antigravity
feeding position are all very positive experiences from the baby's
perspective, too.
